For all benefit-eligible employees, your employer is providing you with 150% of your base annual earnings (annual salary) up to a maximum of $150,000 of life insurance at no cost to you.
Additionally, you may elect Voluntary Life/AD&D Insurance up to a guaranteed maximum benefit up to $150,000 (with NO evidence of insurability) and a maximum benefit of $500,000 (WITH evidence of insurability application).
